Birth Certificate Information for Homebirth in Georgia
In 1993 the Department of Human Resources (DHR), with the assistance of the Georgia Friends of Midwives and the Georgia Midwifery Association, finalized the Rules and Regulations for Vital Statistics for Out-of-hospital birth certificates in Georgia. If you are planning a homebirth in Georgia you will need to have (or your midwife will need to give you) the following documentation to take into your county registrar to file for an Out-of-Hospital birth certificate.
Mother and father will need to bring a picture ID, and 2 bills in your name with your address from the month the baby was born, so they can verify your address at the time of the birth. Remember to notarize the 3 affidavit pages. Without this documentation parents will need to obtain a court order for a birth certificate after a planned homebirth in Georgia.
